Referrals
Step 1
You, or someone on your behalf, should contact your local authority to request a social care assessment. This is to determine your eligibility for care under the 2014 Care Act. The local authority will then conduct a financial assessment to establish funding.
Step 2
Please complete our electronic referral form below or alternatively download the form here. Ensure all mandatory fields are filled in and send it to info@rbsuitesupportedliving.co.uk with ‘referral’ as the subject line. This is typically done by a social worker or a representative from the local authority.
Step 3
Our manager will contact you to arrange a more detailed assessment. Ideally, this will involve other representatives, family, and friends, so we can get to know you better and determine how we can best support you
Step 4
We will coordinate with the local authority regarding our assessment and await funding approval. Once approved, we will arrange a visit and move-in date to your chosen accommodation and commence an individualised care plan.
